Name/Address Change - Cemetery Oversight

Registrants and permit holders are responsible to notify the Office of Cemetery Oversight (office) of any name change or change of mailing/e-mail address. Changes can be made by submitting the Office of Cemetery Oversight Change Request Form (Word).

Name Changes

If you change your name, you must provide a copy of the appropriate court document or marriage license and e-mail/mail it to the office at the addresses provided above.

Failure to notify the office of address changes in a timely manner can prevent registration renewal notification. A late fee ($100 for cemeteries and monument dealers and $200 for crematories) will be charged when a registration is not renewed by the expiration date.
