I do hereby affirm under penalty of perjury that it is my obligation to teach the above named applicant the practical and theoretical practice of barbering authorized by said apprentice registration in accordance with the Laws and Regulations of the Board and the provision set forth under Title 4, Business Occupations and Professions Article of the Annotated Code of Maryland. |

I will: |
(a) |
File a monthly report stating the progress of the training and identify the designated Master Barber supervising this apprentice; |
(b) |
Advise the Board, immediately in writing, when the apprenticeship is discontinued, temporarily or otherwise; |
(c) |
Assure that the apprentice is limited to and performs only the services authorized by this registration; |
(d) |
Assure that the apprentice, when applicable, attends English comprehension courses, during the apprenticeship, if English is not the apprentice's first language or reading comprehension courses, during the apprenticeship, if the apprentice is dyslexic or is reading impaired. |
(e) |
Assure that the apprentice, when applicable, attends courses to help the hearing impaired individuals to understand directions, explanations, or needs of examiners and patrons; |
(f) |
Assure that one properly licensed Master Barber is responsible for only one apprentice at any given time during the apprenticeship training; |
(g) |
Assure full time employment of not less than 30 hours a week to fulfill the training program: |
(h) |
Assure that the apprentice learns all aspects of the barber industry, to include the Maryland Barber laws and regulations. |
(i) |
I understand the Shop/Master Barber will receive the required curriculum and blank reporting forms to be used during this training program from the Board office once this application has been processed and the license has been issued. |
