Information or Help Request - Division of Occupational and Professional Licensing

Formerly Virtual Appointments

The Division of Occupational and Professional Licensing has made changes to our former virtual appointment system that will allow our staff to better assist you with any questions, concerns or issues that you may have and provide you with more options to find the resources you need. In addition to virtual appointments, you can now request to receive information or assistance from our staff by phone or email. We have also improved our virtual appointment scheduling to provide you with more flexible availability windows when requesting a virtual appointment.

You can find the form to submit an information or help request at the bottom of this page.

  1. If you select "Phone" as your chosen method, you will be called by a member of our staff during the time window selected. If we cannot reach you, a voicemail will be left with your requested information or an alternate method will be used.
  2. If you select “Email”, a member of our staff will provide you with any information relevant to your request to the email provided in the form.
  3. If you select "Virtual Appointment"  as your chosen method, we will reach out to you by email with an available time in your given window. Virtual appointments are held using Google Meet. If you plan to join the appointment from a computer browser, you do not need a Google or Gmail account. If you plan to join the appointment from a mobile device, you will need to be signed in with a Google or Gmail account.

Note: Please be as detailed as possible when filling out the form below. The more information you provide to us about your specific needs the more assistance we can provide.

If you need to visit our offices in person to review or drop off documents, please schedule an appointment for our 1100 N. Eutaw Street office online.
