Access to this application is controlled by an on-line password management system. To use this system:
- You must have the License Registration Number that was assigned by the Department of Labor (LABOR) to access this application.
- First Time Users: Login using your LABOR-assigned PIN. You will then be prompted to create a new password of your choice.
- Returning Users: Login using the password that you created on your previous visit. Your LABOR-assigned PIN is no longer valid.
- If you renewed your license after October 2, 2008 and have not completed a peer review worksheet, please sign in and answer the question that best describes your situation. Completion of the peer review worksheet is an addendum to your most recent renewal application.
