Please complete the complaint form below. The fields that have a red asterisk (*) are mandatory required fields. It is extremely important to supply your email address, as we will be sending you an email confirming the receipt of your complaint. This email will also contain additional instructions for the submission of your documents to support your complaint and will include your case number.
The electronic submission of the complaint form is only the first step in the process. You must still submit the required supporting documents to the Board. These documents will be described in detail in the confirmation email. The supporting documents must be submitted within 30 days from the date of the complaint submission. Failure to submit the required documents will result in the closing of your complaint without action.
