How to use this form
The Complaint Referral Form should be used by local law enforcement agencies when forwarding complaints
to the Department regarding alleged violations of the Secondhand Precious Metal Object Dealers and
Pawnbrokers Act.
This form is a Microsoft Word 2003 Document. Please save the file,
"DLLR SPMG COMPLAINT REFERRAL FORM TEMPLATE". Leave it blank for future use. Use the "Tab" key to
navigate through each data field. Each time you complete the form you can save it as unique MS Word
document. Law enforcement officers can then organize the form, supporting investigation reports, scanned
documents, and photographs according in any way desired.
The Department will accept complaint referrals through regular mail or through e-mail. When filing by e-mail,
please attach the complaint referral form, the formal investigative report, scanned documents of daily transaction
reports or other evidence, and, when relevant, photographs. Please use the following e-mail address when referring
complaints: dllrpmcomplaints@dllr.state.md.us. Law
enforcement agencies preferring to file a complaint by regular mail should send it to the following address:
